Portsmouth Families in Mind
Our Families in Mind Project aims to support parents in the transition to parenthood, improve parents emotional well-being and early relationships.
The team includes Specialist Health Visitors, Community Nursery Nurses and a Mental Health Nurse who aim to deliver evidence-based support in a flexible and adaptable way to help meet your needs.
Our service aims to support families experiencing mild to moderate difficulties who would like to improve coping and overcome challenges. We offer services to parents in Portsmouth prior to having their baby, once their baby is born and until their child is two years old.
You can self-refer for the Families in Mind service by logging into your Family Assist account and completing the online self-referral form. A member of the team will contact you within two weeks to discuss your referral and how they can support you. Please Note: You must be registered and logged in to access this form.
Or if you prefer, your midwife, health visitor or mental health professional can support you with a referral to the service.
